The OnTrackNY / Connections Program

The OnTrackNY / Connections Program is part of the Washington Heights Community Service. It is an innovative, unique program for adolescents and young adults who recently have had unusual thoughts and behaviors or who have started hearing or seeing things that others do not. OnTrackNY / Connections helps people achieve their goals for school, work, and relationships. Its mission is to provide premier, evidence based services such as psychiatric treatment, employment and educational support, substance abuse treatment, and family education and support, for people and their families. The clinic offers care from a specialized team for up to 2 years, based on clients’ needs and preferences. The team consists of a psychiatrist, nurse, social workers, mental health clinicians and an employment specialist, many of whom also have appointment at the Columbia University Medical School.

In the words of our program participants: “They actually pay attention to the whole me”; “I can really open up to them and be myself. I feel like I can tell them anything I would tell my best friend.”
OnTrackNY / Connections Program is located at:
1051 Riverside Drive, room 1701
NYC, NY 10032
