HEALTH BENEFITS
ELIGIBILITY:

Must work at least half time for no less than six bi-weekly payroll periods. Eligible dependent enrollees spouse, domestic partner, dependent children to age 19, or age 25 if full time student.

FLEX SPENDING ACCOUNTS:

Dependent Care (800) 342-8017 - Pay for your dependent care and health care expenses with pre-tax dollars.
Health Care (800) 358-7202 - Set aside from $150 to $3,000 to pay for eligible health care expenses that are not reimbursed through your health insurance benefit plans. Must work at least half time.
 

 

NEW YORK LONG TERM CARE INSURANCE:

NYPERL - New York State Public Employees and Retirees Long Term Care Insurance Plan (NYPERL) provides long term care for assistance with personal daily activities. Must work at least half time to be eligible. No medical underwriting for newly eligible employees enrolled with 60 days for date of eligibility.

HEALTH OPTION PROGRAM:

Exchange up to three days of sick leave credits per year for up to $310.50 which will be applied toward the cost of your health insurance premium. Must work at least half time and be enrolled in the health insurance program. Program may continue in 2003.

NYS PENSION PROGRAM:

Three percent (3%) biweekly contribution. Mandatory for full time permanent employees. Optional for all others. Early retirement at age 55 with 5 years membership.

SURVIVORS BENEFITS:

Up to three (3) times salary depending on age at death and enrollment in the NYS Pension Program.
 

SUPPLEMENTAL BENEFITS:

NYS Deferred Compensation Plan administered by Copeland Associates. Set aside up to $11,000 a year ($12,000 year 2003) tax deferred. Payroll Deduction.
 

OTHER INSURANCE

OFFERED THROUGH THE PEF MEMBERSHIP BENEFIT PROGRAM:
Premiums paid through payroll deductions.

$15,000 Accidental Death and Dismemberment Insurance.
